How to Send in Referrals:
We accept physician referrals via fax, phone, or direct secure messaging through your EMR system.
Please download the fillable PDF below and send in via fax or EHR direct messaging:

Phone Referral: 205-271-8000 and press option 2.
Schedulers are available Monday- Friday, 8:30 AM – 5:00 PM
To ensure we are able to give the best patient care, please fill out our referral form completely and send over the following information:
- Office Note (last 2 visits)
- Medications
- Allergies
- Labs (last 2 visits)
- Diagnostics (last 2 visits)
- Family History
- Demographics
- Problem List
- Height/Weight
Sending PHI via EMR Platform:
If you use your electronic medical record system’s direct messaging function to send us referrals, summaries of care, and other patient updates, you can securely send messages and information to a specific provider using their individual direct messaging address or send directly to our practice address listed below.
